Easy Registration

Patient Registration
Go to the DocNutra.com registration page and create an account. If you know the name of the clinic or health provider that referred you, enter it into the "Referring Clinic / Provider Name" field. If you don't have a referral, simply leave it blank.

You can ignore the "NPI Number if you are a Health Provider" field if you are a patient.

Clinic | Health Provider Registration
Go to the DocNutra.com registration page and create an account. Leave the "Referring Clinic / Provider Name" field blank.

Enter your "NPI Number if you are a Health Provider", or email your documentation to store@docnutra.com. Once we have verified your registration information, you will be upgraded to a clinic wholesale account.

You will then access the wholesale pricing exclusively available to Health Providers.
